Opportunity Information

Sutter Medical Group is seeking a dedicated and compassionate Skilled Nursing Facility Physician to join our post-acute care team. This position focuses on providing high-quality, patient-centered medical care to patients residing in skilled nursing and rehabilitation facilities. The ideal candidate will have experience managing medically complex, post-acute, and long-term care patients, with a strong commitment to continuity of care, interdisciplinary collaboration, and optimizing patient outcomes during transitions of care.

 

Responsibilities:

  • Provide comprehensive medical assessments and ongoing management for patients admitted to skilled nursing facilities for post-acute, rehabilitative, and long-term care.
  • Manage and coordinate treatment for patients with multiple chronic illnesses, acute conditions, and complex medication regimens.
  • Conduct daily or scheduled rounds in assigned SNFs, ensuring timely evaluation and management of acute changes in condition.
  • Collaborate closely with facility staff, nursing teams, rehabilitation specialists, social workers, and case managers to deliver coordinated, patient-centered care.
  • Communicate effectively with families, caregivers, and facility staff regarding patient goals, treatment plans, and care transitions.
  • Ensure compliance with all SNF regulations, documentation standards, and quality measures, including participation in care plan meetings and facility quality improvement initiatives.
  • Partner with hospitalists, primary care physicians, and specialists to support smooth transitions of care and reduce hospital readmissions.
  • Participate in quality improvement, population health, and value-based care initiatives related to post-acute and long-term care.
  • Maintain accurate and timely documentation in the electronic medical record (EMR) system in accordance with Sutter Medical Group and regulatory requirements.

Qualifications

  • Medical degree (MD or DO) from an accredited school of medicine.
  • Board certification in Internal Medicine or Family Medicine (Geriatric Medicine certification preferred).
  • Active and unrestricted medical license in California.
  • Demonstrated experience in post-acute, skilled nursing, long-term care, or geriatric settings.
  • Strong clinical skills in managing complex, multi-morbid patients.
  • Excellent communication and interpersonal skills, with a patient- and team-centered approach.
  • Ability to work effectively in an interdisciplinary, facility-based environment.
  • Proficiency in electronic medical records (EMR) systems.

Preferred Qualifications:

  • Fellowship training in Geriatric Medicine or experience in post-acute/rehabilitation medicine.
  • Knowledge of CMS regulations, SNF billing and documentation requirements, and quality metrics.
  • Experience participating in value-based care or population health programs.
