Sutter West Bay Medical Group (SWBMG) is seeking a Palliative Medicine Physician in our Advanced Organ Therapy (AOT) Clinic. Work in one of the most desirable cities in the United States in close proximity to Silicon Valley with easy access to beaches, mountain recreation, the Sonoma wine region, and world-class restaurants.   • Location: 1100 Van Ness Ave, San Francisco, CA
  • Opportunity details:
  • Medical Care Delivery: Opportunity to provide outpatient Palliative Medicine evaluation and follow-up care for patients being followed in the AOT clinic for either liver failure, heart failure, or advanced GI malignancy, including pre-transplant evaluation when required. There is an option for inpatient coverage when patients are hospitalized, if desired for continuity, and patients may also be referred to our robust inpatient PC team.
  • Goal Support Care: Facilitate discussions regarding goals of care and advance care planning, assist with complex care coordination and provide support with medical decision-making for patients and their families.
  • Symptom Management: Provide pain and symptom management support for established AOT patients.
  • Team Integration: Join a well-established Palliative Medicine department, with a rich 15+ year history, which now includes 6 physicians across 3 hospital campuses and outpatient cancer services and an inpatient Palliative Care Licensed Clinical Social Worker.
  • Interdisciplinary Collaboration: Collaborate extensively with multi-disciplinary AOT team, community physicians, social workers, chaplains, and administrators.
    Educational Opportunity: Engage in resident education with opportunity to participate in research.
  • Technological Interface: Use an integrated EMR system (EPIC) across both the clinic and hospital.

Qualifications

  • ACGME Fellowship Training in Hospice and Palliative Medicine, Board Certified or Eligible
  • Board Certified in Internal Medicine or Family Medicine.
  • At least 2 years of post-fellowship clinical practice in Palliative Medicine, including outpatient experience, strongly preferred.
  • Experience working with Transplant program preferred.
